This state does not post this one centrally — the court that heard your case hands it out, and the wording differs by county. Ask the clerk of that court for its current version.

What a court checks before this one is granted

The case ended in a dismissal, an acquittal or a no bill, so there is no waiting period in Ohio — it can be filed straight away.
Filed in the court that heard the case under ORC 2953.52, which is a different section from the conviction route.
Ohio posts no statewide form for it; the court's own version is the one to use.
